Cell Phone - what would you do?

I work at a coffee shop. Lets pretend you do too. What would you do in the following situations?
1. If someone walks up to you already on their cell phone and whispers their order to you in a barely audible voice then automatically returns to their conversation?
Would you still say the "is there anything else? room for cream? thank you very much have a great day stuff?" or just get them their coffee and stick your hand out for money?
2. What would you do if someone walks up to your counter THEN right when you go to ask them what you can get them, dials the phone and proceeds to talk and expect you to wait on them.
This actually happens more than you would believe. I usually just go silent and admittingly probably seem pretty rude/cold. But other employees acctually walk away and don't acknowledge the customer until they get off their phone, especially in the latter situation.
What would you do (what should I do!)?

1. Just give them the coffee and take their money. If it's not exactly what they wanted, they'll have to get off the phone and order properly.
2. Start helping the next person in line.

So its not seen as rude if I don't go through the "have a great day!" speil with people on their phones?

You can say it, but they're not listening anyway.

Overall I'd hope management would put up a sign and give the baristas the joy of declining to help anyone on the phone...but as that doesn't seem to be the case
1-I'd probably be sent to e-hell as I'd be passive-aggressive and ask them the questions about what they'd like and how they prefer their coffee in the way most likely to interrupt their phone call.
2-I'd go to the next person in line, with the statement of "ma'am, until this gentleman is done with his conversation, I can't help him, so what could I get for you?"
(I don't think it's rude to skip the "have a great day" bit when folks are clearly distracted.)
