I suppose it depends on what happens if the report is wrong.  If you're not responsible for the information in the report just ensuring that it is compiled and sent.  Start with an email to everyone who sends you data stating that you do not have time to check their numbers, but in the future will report the numbers you are sent.  Then just send it in with the number he sends you and make it clear that the report is based on the information he sends you.

If you are responsible, document each interaction and send it back to him after you find the first error.  Co-worker, I was reviewing your report and found that number 2 was wrong (don't provide the right number),please send me the correct numbers, and copy his boss on that.  Next time; co-worker, I see you updated number 2, but now number 3 is wrong, please send the correct report, and copy his boss.  Repeat until the correct report is sent.
