That's a tough one.

It really depends on your friend, and how well she would take something like that. Alot of people would be very offended, but sometimes, when you have an extremely close friend, they would take it as you were looking out for them, rather than an insult.

I would judge it by your friendship, how close you are, and how her personality is.

Just remember, sometimes it's better to have a mad friend, and help them, then have a great friend, and watch them hurt themselves.

Absolutely don't say anything, and that includes the possibly-worse stuff like, "let's go to the gym together for a fun time."  Eek.  Everybody thinks they are so subtle about stuff like that, but the more she politely pretends not to get the subtext, the more thickly people pile it on.   

I do think you're okay to bring up the son's health, but in such a way that you two friends are a unit discussing something together (not you vs them).  Bring it up only like you'd bring up any other kid topic.
