Mispronunciations that Have Gotten You in Trouble

I have two friends who were trying to set me up with a guy friend of theirs on New Year's Eve. We were casually chatting, and as my friend Mark and I had gone to elementary school in the same small town that he and his friend had graduated from, I asked the guy what his last name was. The guy's last name is Russian, and it's pronounced "Chur -ko". Unfortunately, my mouth was moving faster than my brain, and I because I hadn't heard him right, I said "Your last name's Jerkoff?" I'm thinking man, he must have been picked on as a kid.

err, posted before I was finished writing.
So anyway, it was only later that I found out what his last name really was.... which he didn't care about, cause his friends called him "Jerkoff" as a joke (and no, he is not a jerkoff - he';s a very decent guy). Has anyone else accidentally mispronounced something, only to be really, really embarassed later?
